1. Professional and Leadership Disclosures

Center for Men's Health Rhode Island is committed to accurate representation of the qualifications, authority, and responsibilities of its clinical and administrative leadership.

• Leonard Berkowitz, PA-C: Chief Executive Officer and a licensed physician assistant. Clinical services that he provides are limited to his professional education, training, licensure, authorized scope of practice, and independent clinical judgment.
• Jose Bolanos: Co-Founder, Chief Operating Officer, Health Lead, and Healthcare Systems Architect. His responsibilities include operations, healthcare technology, organizational systems, patient education support, research-related work, privacy-conscious workflows, and administrative compliance. He is an internationally educated physician who does not hold an active United States medical license. He does not independently diagnose conditions, prescribe medications, or provide healthcare services that require an active United States professional license.

Clinical decisions are made only by an appropriately licensed healthcare professional after an individualized evaluation. Administrative, educational, research, and technology roles do not independently confer clinical authority.

2. Pharmacy and Pharmaceutical Supplier Disclosures

CMH-RI may work with independent pharmacies and pharmaceutical suppliers in connection with provider-directed prescriptions or clinical supplies. Pharmacy selection, medication availability, dispensing, shipping, pricing, insurance coverage, and processing times remain subject to applicable law, clinical appropriateness, patient choice, pharmacy policies, and supplier availability.

LegitScript certification: CMH-RI maintains LegitScript Healthcare Merchant Certification. LegitScript is an independent certification organization. Certification is not a medical license, government approval, endorsement of a particular treatment, or guarantee of any clinical outcome.

3. Medical and Educational Disclaimer

Information on this website is provided for general educational and informational purposes. It is not a substitute for an individualized medical evaluation, diagnosis, treatment plan, or advice from a qualified healthcare professional who knows the patient's medical history and current condition.

No provider-patient relationship: Viewing this website, sending an email, completing a general contact form, or requesting an appointment does not by itself establish a provider-patient relationship. A clinical relationship begins only after an authorized clinician accepts the individual for care and completes the required evaluation, consent, and intake process.

Individual clinical decisions: Eligibility for evaluation, laboratory testing, medication, treatment, or another clinical service depends on the licensed clinician's assessment of the patient's history, examination when indicated, diagnostic information, risks, potential benefits, alternatives, and applicable professional standards.

Off-label use and developing evidence: A licensed clinician may, when legally and medically appropriate, discuss or prescribe an FDA-approved medication for a use that is not included in its approved labeling. Off-label use is not the same as an investigational product. When applicable, the treating clinician will address material risks, potential benefits, reasonable alternatives, and the limits of available evidence through the clinical consent process.

No guaranteed outcomes: Responses to healthcare services vary. CMH-RI does not promise or guarantee a particular diagnosis, treatment response, result, duration of benefit, or timeline.

4. Telehealth and Professional Licensure

Telehealth services are available only when clinically appropriate and legally permitted. At the time of the telehealth encounter, the patient must be physically located in a jurisdiction where the treating healthcare professional is authorized to provide the requested service.

Telehealth availability may depend on the patient's location, the clinician's licensure and scope of practice, the nature of the medical concern, applicable prescribing requirements, technology limitations, and whether an in-person evaluation is clinically necessary.

5. Privacy, Website Forms and Health Information

General website forms and ordinary email are intended for non-urgent administrative communication, such as appointment requests and general questions. Please do not include detailed medical histories, photographs, identification documents, payment-card information, portal credentials, or other sensitive health information unless CMH-RI specifically directs you to an approved secure method.

Established patients should use the CMH-RI patient portal for clinical questions, refill requests, laboratory inquiries, and transmission of health-related information:

Use of health information is governed by applicable privacy and security requirements and CMH-RI's Notice of Privacy Practices. No internet transmission or electronic storage system can be represented as completely free from operational or cybersecurity risk.

6. Side Effects and Adverse Event Reporting

Patients should promptly contact the prescribing or treating clinical team concerning unexpected symptoms, suspected side effects, medication errors, or other treatment concerns. Severe or rapidly worsening symptoms may require emergency evaluation.

Patients and healthcare professionals may voluntarily report suspected adverse events involving FDA-regulated medical products through FDA MedWatch or by calling 1-800-FDA-1088. Reporting an event to FDA does not replace contacting the treating clinician or obtaining emergency assistance.

7. Permitted Website Use

You may use this website only for lawful personal and informational purposes. You may not:

• Attempt to gain unauthorized access to any website, account, server, portal, database, or network.
• Introduce malicious code, viruses, automated attacks, or other material intended to disrupt or compromise systems or data.
• Impersonate CMH-RI, a clinician, an employee, a contractor, or another person.
• Use automated collection methods in a manner that violates applicable law, bypasses technical controls, places an unreasonable burden on the website, or collects protected or confidential information.
• Use website content to make false, misleading, or unauthorized representations concerning CMH-RI or its services.

8. Intellectual Property

Website content, branding, original written material, design elements, software, graphics, and other materials are owned by CMH-RI, its affiliated rights holders, its licensors, or the respective third-party owner, as applicable.

CMH-RI does not control every third-party website and is not responsible for its availability, content, privacy practices, security practices, pricing, services, or terms. A link does not necessarily constitute an endorsement of every statement, product, or service offered by that third party.

10. Website Availability and Limitation of Liability

CMH-RI makes reasonable efforts to maintain accurate and useful website information but does not warrant that every page will always be complete, current, uninterrupted, or error-free. Website content may be corrected, changed, or removed without prior notice.

To the fullest extent permitted by applicable law, CMH-RI and its authorized representatives will not be liable for indirect, incidental, special, exemplary, punitive, or consequential damages arising solely from use of, or inability to use, this website.

These website terms are governed by the laws of the State of Rhode Island and applicable federal law, without regard to conflict-of-law principles, except when another law must apply.

Subject to any mandatory legal right to bring a matter in another location, proceedings concerning these website terms shall be brought in a court of competent jurisdiction serving Providence County, Rhode Island.

If a provision of these website terms is determined to be invalid or unenforceable, the remaining provisions will continue to apply to the extent permitted by law.
